St. Charles Health System in Bend, Oregon is accepting applications for a full time, Director of Care Coordination position. The Care Coordination department includes Nurse Navigation, Case Management, Social Work and Spiritual Care.
The Nurse Navigation department engages in a collaborative process between the inpatient and outpatient care teams in conjunction with the patient to address mutual care goals, remove barriers to care, coordinate efforts between multiple care providers and encourages patients to individually manage their care. The work is characterized by advocating for patients to receive the right care, at the right time by the right provider and promotes quality and cost-effective interventions and outcomes.
The Case Management department engages in a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet the client's health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost- effective interventions and outcomes.
The Social Services Department provides many services to our multi-hospital organization including: working with a multi-disciplinary team to assess needs of patients and families, provide appropriate resource referrals, provide supportive counseling as needed, coordinate discharge planning and provide other identified services as needed to support those affected by chronic or acute illness.
The Director of Care Coordination assumes responsibility, authority, and accountability for patient care activities and service operation, provides strategic and operational leadership in the organization as assigned and has specific management authority responsibility for assigned campus services and/or departments. In addition, the Director of Care Coordination develops and implements the operational strategies for the transitions of care across the continuum for St Charles Health System. The program creates a coordinated infrastructure with standardized workflows, evaluation tools, and metrics to document the management of designated patient populations such as: insured employee population, Medicare/Medicaid clients, specialty specific focused populations (Cancer, Orthopedics, Heart etc.). Care coordination ensures the accomplishment of the organization’s mission, vision, and values, strategic goals and achievement of True North Metrics. This position will oversee the provision of patient health care navigation and care coordination to meet optimal outcomes for patients across the continuum of care.
This position directly supervises the Managers of Case Management/Nurse Navigation and Social Work/Spiritual Care.
Bachelor’s degree in nursing.
Master’s degree, preferably in a health related field or will consider 3 years of Director level leadership experience, and completion of a Master’s Degree program within 3 years of date of hire
Master’s Degree in Nursing
Current RN licensure with the Oregon State Board of Nursing.
Valid Oregon driver’s license and ability to meet St. Charles Health System driving requirements
Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites
AHA Basic Life Support for Healthcare Provider certification
Certification in Nursing Leadership, Case Management, or Utilization Review/ Management
Five years of clinical experience in Nursing.
Previous supervisory or health team management experience.
Five years’ experience with successful management at the director or equitable management level.
Case Management experience or case management department management experience required.
Director level experience leading Case Management/Utilization, Nurse Navigation, and Social Work
Experience with the management of multiple hospital departments.
