Service Director Home Health Hospice (RN preferred)
Martinez, California
Serves as the Administrator of the Home Health/Hospice Agency. Responsible for multiple medical center facilities and manages the operations of Home Health/Hospice departments across Service Areas. Plans, organizes, directs, evaluates and coordinates Home Health/Hospice programs. Directs the day-to-day operations. Works with KFH and TPMG staff developing strategic plans, providing quality care and cost-effective services that are aligned with federal, state, and local regulations.
Essential Responsibilities:
Collaborates with physicians, Directors, and other health care providers identifying and implementing innovative models and best practices, emphasizing service improvements and cost reduction.
Directs development and implementation of quality and utilization standards across the continuum of care to ensure coordinated plans of treatment, customer focused delivery of services and cost-effective utilization of necessary services.
Ensures compliance with administrative, legal and regulatory requirements of the Health Plan contract and governmental and accrediting agencies.
Develops and enhances unit-based standards as identified by regulatory agencies.
Develops a service line that achieves a high level of customer satisfaction with emphasis on customer service and highest standards of quality and innovation.
Utilizes research data to implement clinical changes and the delivery of patient care and member services.
Manages and resolves human resource, labor relations, employee and department safety and risk management issues.
Directs the budget and resource allocations.
Manages the financial performance and identifies and implements strategies to reduce costs and improve quality of care and services.
Kaiser Permanente conducts compensation reviews of positions on a routine basis. Such changes are generally implemented only after notice is given to affected employees.
Basic Qualifications:
Minimum five (5) years of management experience of which 3 years must be in HHH.
Bachelor's degree in nursing or other related field.
Demonstrated knowledge of Nurse Practice Act, The Joint Commission, and other local, state, and federal regulations.
Knowledge of federal, state and accreditation standards applicable to Home Health/Hospice required.
Knowledge of governmental and other regulator standards, requirements, and guidelines related to quality improvement.
Strong working knowledge of ongoing monitoring techniques (including criteria development and statistical analysis); care delivery in home care; total quality management principles, tools and techniques.
Effective communication, negotiation and leadership skills.
Demonstrated strong interpersonal communication skills.
Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
Master's degree preferred.
