CHANS Home Health & Hospice is full service non-profit, Joint Commission Accredited agency providing skilled home health, hospice, and an array of community-based wellness clinics and support services in Midcoast Maine. Part of Mid Coast–Parkview Health, CHANS has been providing home health and wellness services in our community since 1947. When you or a loved one is sick, recovering from surgery or an acute or chronic illness, or approaching the end of life, there is no place like home. We provide medical services in your home, where you are most comfortable.
Manages the patient and employee activities of the Skilled Nursing or Hospice department under the direction of the Senior Director of Clinical Services. Demonstrates commitment to agency’s goals and to the organization’s mission, vision, and values. Acts as an advocate for patients and families needing home care services and oversees the patient’s experience and outcomes of home health care.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Bachelors of Science in Nursing is preferred, or comparable years of management/leadership experience required along with minimum of ADN.
License/Certifications: Currently licensed as an RN in the state of Maine.
Experience: Experience working in Home Health or Hospice. 3 Years of Leadership experience preferred.
